Upheal's Golden Thread and Treatment Plans

Connecting your Treatment Plans to every progress note

Appointment types and self-scheduling are in beta, which means not everyone has access to this feature yet, and we're still polishing it up.

Would you like your notes to follow your client's progress session-to-session?

With Upheal's Golden Thread, your notes reference your client's ongoing progress in relation to their treatment goals.

How does it work?

First, you generate a Treatment Plan. This can be based on up to three sessions you've held with your client. Once you've created a Treatment Plan, your progress notes will refer back to the plan's treatment areas, objectives, and interventions.

Please note: Golden Thread only works with our new Treatment Plans.

We're working on a simple way to move existing plans from our old system to the new one. For now, if you'd like to use Golden Thread, please create a new plan in the Treatment Plan (New) tab.

Which note templates work with Golden Thread?

Currently, these note templates can connect to your Treatment Plan:

  • SOAP

  • Enhanced SOAP

  • Upheal (our own template)

If you make custom note templates, the individual sections from these note types can also refer to your Treatment Plan.

We're actively working on incorporating more note templates with Golden Thread.

Creating a Treatment Plan

Tip: To make generating a Treatment Plan faster and easier, we recommend capturing an intake session with your client first.

  1. Go to your client's profile.

  2. In the Treatment Plan (New) tab, click Create Treatment Plan.

  3. First, we'll generate a Case Snapshot, which provides essential information about your client and their treatment.

    Choose up to 3 sessions on which to base your Case Snapshot, and select modalities.

    (You can also write the Case Snapshot on your own β€” Click Enter Case Snapshot manually.)

  4. Once you've chosen sessions, click Continue.

  5. Review the Case Snapshot, and make changes as needed.

  6. Scroll down, and click Generate plan.

In a few minutes, your Treatment Plan will be ready for review. New sessions you hold with this client can now refer to their Treatment Plan.

Navigating your Treatment Plan

What do Treatment Plans include?

  • At the top of the page, you'll see the Case Snapshot. You can click to expand it.

  • The Treatment Plan has multiple treatment areas. Each treatment area has multiple objectives, each with interventions.

Editing Treatment Plans

  1. To make changes, click Edit plan.

  2. You can:

    1. Type to edit text

    2. Add objectives and interventions

    3. Change or remove stages

    4. Add treatment areas (scroll to the bottom)

  3. Once you're finished, scroll down to the bottom and click Edit treatment plan to save.

Exporting Treatment Plans

To export your Treatment Plan, you can copy or download it.

To copy your Treatment Plan, click the clipboard icon.

To download it as a PDF, click the PDF icon.

Deleting Treatment Plans

To delete a Treatment Plan, click Delete Treatment Plan at the top of the page, and confirm.
