Signing off on progress notes

You can sign off on progress notes digitally in Upheal, making it quick and easy to prepare your notes for insurance and legal purposes.

Important: Signed notes are locked

When you sign off on notes, the notes become locked, and you won’t be able to make changes without first unlocking them. If you do unlock the notes, you will be asked to provide a reason for unlocking, and your provided reason is recorded in the HIPAA audit log.

Signing off on notes

  1. From your client’s page or your Upheal dashboard, click on the session in which you want to sign off on notes.

  2. In the Progress notes section, click Sign off.

  3. To sign, click Sign off and lock.

Your notes are now signed and locked. If you download the notes as a PDF, you’ll see your signature and the date you signed the notes at the bottom. If you want to edit your notes, you’ll need to unlock them.

Unlocking notes for editing

You can unlock your signed notes for editing. You’ll be asked to provide a reason for unlocking and editing the notes, which will be recorded in the HIPAA audit log.

  1. In the Progress notes section, click Unlock.

  2. Read the message, and enter a reason for unlocking the notes.

  3. Click Accept & Unlock.

You can now make changes, and re-sign your notes any time by clicking Sign off again.

Downloading your notes as a PDF

In the Progress notes section, click on the PDF icon. Your download will start immediately, and the file will be saved to your downloads folder. Your PDF document will include your digital signature and a timestamp at the end of the notes.
