If you'd like to collect client consent in your name rather than Upheal's, that's no problem. Go ahead and use your own Privacy Policy (more info here).

If you don't have your own Privacy Policy form, but would still like to keep things between you and your client – feel free to use our template. Just make a copy of this file, fill in your details, and modify it based on your needs.

Do remember: Your client still needs to approve Upheal’s Terms of Service to use Upheal. It is a one-time consent that every client or patient approves before they join their first session.
